Registration Packet = Contains the following: ALL required documents, a printed copy of your online registration, and all required personal documents together and ready for submission to your school site.
Online Registration Application = The registration platform WPUSD uses to enroll children in our schools.
Personal Documents = Documents specific to each child (i.e. birth certificate, immunization records, etc)

We will be accepting registration for the 2021-2022 school year as normal.

WHAT DO I DO?
Please follow the steps below to register your child. When turning in registration packets, please note that face coverings are required at all times while on school campuses.

Please read the information and follow all four (4) steps below to register your child. FOR 2021-2022 REGISTRATION: All completed registration forms, along with supporting documents, may be turned in to your child’s home school (confirm child's home school here). For those registering at Twelve Bridges High School (TBHS), please turn your packets in to Lincoln High School, as the office at TBHS is not open yet. Packets must be submitted in-person, not mailed. For special accommodations regarding COVID-19, please contact your child's home school.
Parents registering for FIRST GRADE ONLY may request their child participates in the Bilingual Immersion Program. This selection can be made in the Online Registration Application. Those who have turned in their complete registration packet by the due date (2/19/2021) will be placed in the selection pool and chosen based on a lottery (not first come, first serve). Any registration packet turned in after the due date will be placed on a space available wait list. We will make every effort to accommodate all requests, but cannot guarantee requested placement. For more information regarding specific requests, please contact your child’s home school upon completion of registration.

2021-2022 Registration Process
Please complete the required steps outlined below. If you need assistance completing forms, please contact your child’s school. If you are having technical difficulty accessing the online registration system, please contact the Technology Department at helpdesk@wpusd.org. We are happy to help!
Step 1 - REQUIRED FORMS:
Complete each form below. Please note, these documents are REQUIRED to be completed and turned in as part of your complete Registration packet.
- Residence Verification Form - English | Spanish
- Special Programs/Health Concerns Form English/Spanish - this form is required, even if your child does not participate in any Special Education Programs
* If applicable for your student, please fill out the following forms and submit with the forms above.
- Medication Administration Form - English/Spanish
- Medical Special Accommodations Form - English | Spanish
Step 2 - Online Registration Application:

Step 3 - Required Personal Documents:
- Proof of Age: Any of the following are acceptable - Original or certified copy of birth certificate; statement by the local registrar or county recorder certifying the date of birth; baptism certificate; passport, or when none of the foregoing is obtainable; an affidavit of the parent, guardian or custodian, or any other appropriate means of proving the child's age
- Proof of Residency: Any of the following are acceptable - Property tax payment receipts; rental property contract, lease, or payment receipts; utility service contract, statements, or payment receipts; pay stubs; voter registration; correspondence from a government agency; declaration of residency executed by the parent or legal guardian of the student.
- Proof of Immunizations: Copy of Immunization Record
Required Immunizations:
- DTP - 5 doses (4 doses are acceptable if the 4th dose has been given on or after the 4th birthday)
- Polio – 4 doses (3 doses are acceptable if the 3rd dose has been given on or after the 4th birthday)
- Hepatitis B – 3 doses
- MMR – 2 doses
- Varicella – 2 doses
Step 4 - Turning In Your Completed Registration Packet:
- Be sure your Registration Packet is complete
- TURN IN YOUR COMPLETE REGISTRATION PACKET TO YOUR CHILD'S HOME SCHOOL (click here to find your home school). **For those registering at Twelve Bridges High School - please turn in your registration packet to Lincoln High School**
FAQ's
Q: Will there be an option for distance learning for the 2021-2022 school year?
A: At this time, there is not a set plan for distance learning in the fall. It is our hope we are able to offer a regular in-person experience district-wide. As the current situation is ever-changing, we will continue to adapt and plan for the 21-22 school year.
Q: When can I begin the registration process?
A: Online registration opens Monday, January 25, 2021 at 8:00am.
Q: Where do I turn in my registration packet once I complete all 4 steps?
A: Packets may turned in to your child's home school beginning Monday, January, 25, 2021.
Q: Do I have to turn in my complete packet by a certain date?
A: YES! New student registration packets must be turned in by Friday, February 19, 2021 at 4pm. *Note: We will be accepting registration packets after this date, but we need to gather enrollment numbers for staffing purposes.
Q: What if my registration packet is incomplete? Can I still turn it in to reserve a spot for my child?
A: No, incomplete applications will not be accepted.
Q: What if I do not want my child to attend our designated home school?
A: You may submit an intra-district transfer request using the online request form available on the district website. Intra-district transfers are for one year only and families must reapply each year. Note: You still need to turn in your packet to your child’s home school. Questions regarding INTRA-district transfers may be directed to your child's school.
