All the required documents for registration that you will need to complete are provided below as a reference and are also included within the online registration. If you need assistance completing the online forms, please contact your child’s school. Step 1 - registering your student online:
Please use the online registration link below to complete your child's pre-registration form. PLEASE NOTE: After completing the pre-registration, your child's school site will email you with further instructions on completing the registration process. This could take up to two days to process and approve your pre-registration.
If you currently have students in the District and previously created a Parent Portal account, after you complete the pre-registration below, you will receive an email with your new student's Access ID Code. This could take up to two days for the site to process and approve your pre-registration.

STEP 2 - REQUIRED FORMS:
Complete each form below. Please note, these documents are included within the online registration (Step 1). They are also listed below for your convenience. These forms are required and need to be submitted within the online registration or turned in to your child's home school (for submission instructions, see Step 4).
- Residence Verification Form - English | Spanish
- Special Programs/Health Concerns Form English/Spanish - this form is required, even if your child does not participate in any Special Education Programs

- Medical Special Accommodations Form - English | SpanishSTEP 3 - REQUIRED PERSONAL DOCUMENTS:
Submitting the following documents is required as part registration process. You have the option to upload your documents through the online registration process (Step 1) or turn in copies to your child's school office.
- Proof of Age: Any of the following are acceptable - Original or certified copy of birth certificate; statement by the local registrar or county recorder certifying the date of birth; baptism certificate; passport, or when none of the foregoing is obtainable; an affidavit of the parent, guardian or custodian, or any other appropriate means of proving the child's age
- Proof of Residency: Any of the following are acceptable - Property tax payment receipts; rental property contract, lease, or payment receipts; utility service contract, statements, or payment receipts; pay stubs; voter registration; correspondence from a government agency; declaration of residency executed by the parent or legal guardian of the student.
- Proof of Immunizations: Copy of Immunization Record
Required Immunizations:
- DTP - 5 doses (4 doses are acceptable if the 4th dose has been given on or after the 4th birthday)
- Polio – 4 doses (3 doses are acceptable if the 3rd dose has been given on or after the 4th birthday)
- Hepatitis B – 3 doses
- MMR – 2 doses
- Varicella – 2 doses

STEP 4 - SUBMITTING YOUR REGISTRATION:
Families have two options for submitting your Required Documents (Step 2) and your Personal Documents (Step 3):
1. FULLY ONLINE - You will need to download the Required Forms linked under Step 2, save them to your device, and upload them within the online registration platform. You will also upload your Personal Documents (Step 3) within the online registration platform (you can scan or take photos of your Personal Documents for upload). If you upload all required documents to the on-line registration portal and submit, your registration process is complete.
2. PARTLY ONLINE/PARTLY IN PERSON - You will complete the online portion of the New Student Registration (Step 1). Then you will print the forms under Step 2, fill them out entirely and turn them in to your child's home school. You will also bring copies of your Personal Documents (Step 3) to your child's home school. If you opt to NOT UPLOAD your personal documents to the on-line registration portal you must bring those items into the school office to complete your registration process.
